JOB TITLE: COMMUNITY INCLUSION LEAD

SALARY SCALE: Community Inclusion Lead (Team Leader HSE Consolidated
Pay Scale)

THE ROLE

The Community Inclusion Lead role combines leadership and practice
development with the continuous development of the workforce to ensure
the needs of the organization are met. The aim of the role is to
transform service provision utilizing a Supported Self-Directed Living
(SSDL) Framework of Practice cognisant of National Policy &
Regulations.

You will be committed to supporting people with disabilities to
?live their best lives? in a person centered, community based,
socially inclusive manner in accordance with our core values, national
standards, and underpinned by quality, evidence-based practice and
research.

This role focuses on building the capacity and capability of the
workforce to deliver high quality, safe and effective service
provision rooted in Social Role Valorisation (SRV) Theory of Practice.

THE PURPOSE OF THIS ROLE WILL:

* promote a culture of learning and development within the
organization
* to work in collaboration with the Community Support Teams
* contribute to the dissemination of innovative approaches to
community service provision
* develop relationships with Individuals and their supporters to
garner support, build momentum for change, and identify leaders who
can develop capacity to mentor others
* continuously promote that the Individual is put at the center of
the work, thinking, planning and decision-making
* coach and influence Community Team Members in such a manner that
optimizes and creates the best environment for success of the
transformative work with people
* work in partnership with; and influence key stakeholders from all
service functions, to ensure that they are clear as to their
contribution to the achievement of this critical strategic objective
for the organization.
* use data gathering, analysis, benchmarking, training initiatives,
action plans, evaluation, story-telling and other evidence-based media
to support the achievement of these aims.

manage any additional roles that are under the remit of the Community
Inclusion Lead

QUALIFICATIONS REQUIRED:

* Qualification in Social Care, Social Work or Nursing
Or
* An appropriate qualification in Health or Social Care Management
or other relevant disciplines.

ESSENTIAL SKILLS AND EXPERIENCE

_EXPERIENCE:_

* Passion for innovative solutions and process improvement
* Excellent analytical skills and ability to make sound decisions
with a well-reasoned rationale and to stand by these
* Proven organizational and time management skills to meet
objectives within agreed timeframes and achieve quality results
* Evidence of effective planning including an awareness of resource
management and the importance of value for money
* The ability to lead the team by example, coaching and supporting
individuals as required in the delivery of service excellence
* Strong interpersonal and networking skills including strong
written communication skills
* Must be able to interface at all levels of the organization,
maintain relationships with colleagues and other stake-holders to
achieve results through collaborative working
* Must work co-operatively with and influence senior management
colleagues
* Working knowledge of all relevant legislation and HIQA Standards
and a proven ability to implement standards and regulations.
* Knowledge of IT & AT that supports and enables self-direction

HEALTH

In order to be successful a candidate must be fully competent and
capable of undertaking the duties as above and be in a state of health
such as would indicate a reasonable prospect of ability to render

_PROFESSIONAL KNOWLEDGE & EXPERIENCE_

_DEMONSTRATE:_

* Knowledge of person Centre community-based support
* Knowledge and experience of community development
* Knowledge of Social Role Valorization
* Knowledge of Disability Services, policy and planning processes
* An ability to provide creative and innovative approaches to
current practices and an awareness of current best practices in other
jurisdictions
* A working knowledge of and competence in use of Word, Excel and
Email
